Use: This form is used if you are starting a proceeding in the Court that claims that your employment has been unlawfully terminated.
Filing fee: The fee payable under subsection 644 (2) or (3) of the Workplace Relations Act 1996. Currently this fee is $57.30. You can apply to have this fee waived if payment would cause you financial hardship.
