Practitioners are reminded of the provisions of O41 r3 which set out the details that are to be shown on the first page of a document prepared by a party for use in the court.

The first page is to have a horizontal line drawn at the foot of it, below which are to be shown:

• The party on whose behalf the document is filed
• The name, address, telephone number and facsimile number of the solicitor for the party
• The name, address, telephone number and facsimile number of a town agent (if any)

If the document sought to be filed does not comply with the provision of the subrule, then it may not be accepted for filing.

Handwritten notations which are intended to rectify a defect also may not be accepted.
